Opt-Out/Opt-In deadline

ATTENTION – DEADLINE REMINDER - Graduate Health and Dental Plan(s) – Opt-Out/Opt-Ins WINTER 2013

The health and dental opt-out/opt-in deadline for the Winter 2013 semester is Monday, January 28th, 2013.

1)        OPTING OUT

To opt-out of the GSU health and dental plans: the opt-out forms are at the GSU office and online at:


You can fax, mail, email, or bring them to us in GH-2007. To opt-out you will need to provide proof of alternate insurance coverage (MCP is not sufficient).

NEWS STUDENTS - Please note that this is the only OPT OUT process in place for graduate students.

Please note that as long as your student status doesn't change (you remain a full-time student), you will not need to opt-out again during your program (unless you go from part-time to full-time, take a leave of absence, etc.)

Health and Dental Charges and Refunds

Each semester, all full-time students are initially charged the health and dental fees. The fees are reversed for students who have opted out, generally by 5-6 weeks into a given semester. If you have opted out, you will initially see the charges on your account. You can ignore them (as they should be removed by early February for Winter 2013), or the cashier's office will refund them to you after this date in case you've already paid them.

2)        OPTING IN

To opt-in to the GSU health and dental plans: Opt-in forms can be downloaded from the address above. International graduate students that have opted out of the ISA Health insurance (Student Guard) will be automatically included into the GSU Health insurance unless they opt out before the deadline (see information above).

Students that are returning to the plan after OPTING OUT, leave of absence, change of status, etc, must submit an opt in form, simply paying for the health and dental fees is NOT enough.

3) Family Health & Dental Application

Eligible students are able to extend their coverage to their partners, including same sex partners, and dependent children.  Family members can be added to your coverage annually during the fall semester registration or in your first semester of studies. Please contact our office for further information.

Please note that the family coverage fees are in addition to your regular single student coverage fee.
